Florida Advanced Work Zone - 2014 SUMMARY: I have over nineteen years of construction
experience with ten years of combined MDX and FDOT Heavy/Highway construction
experience. Providing safety management and loss control services for
Bridge & Roadway contractor including multiple subcontractors consisting
of pre-planning developing risk assessments, claims, managing various safety
programs including return to work and safety incentive programs. Managing
the FDOT fleet compliance program, accident investigations, 10 & 30 hour
OSHA outreach training, weekly bilingual tool box talks and site audits.
Recommendations and best practice solutions are issued and enforced in an
effort to improve site conditions, work practices, maintaining criteria’s and
strengthen EH&S programs. Conducting the new hire orientation and
individual employee JHA evaluations, managing crane compliance program.
Assisting in all legal matters including theft investigations assisting state

maintenance of traffic (MOT) inspections. PROJECT EXPERIENCE: I-595 - Corridor Roadway
Improvements - Design Build, FDOT, District 4, Broward County, FL – Safety
Manager - The
I-595 Corridor Roadway Improvements project consists of the reconstruction
and widening of the I-595 mainline and all associated improvements to
frontage roads and ramps from the I-75/Sawgrass Expressway interchange to the
I-595/I-95 interchange, for a total project length of approximately 10.5
miles. The project passes through, or lies immediately adjacent to, six
jurisdictions: City of Sunrise; Town of Davie; City of Plantation; City of
Fort Lauderdale; Town of Dania; and unincorporated areas of Broward County. Completion Date: March 2014; $603 Million I-95 Golden Glades Interchange –
FDOT, District 4, Broward County, FL – Safety Manager -This project will extend the High
Occupancy Toll Express lanes from northern Miami-Dade County north 13 miles
to Fort Lauderdale. The project includes installing Intelligent
Transportation System components; modifying the Ives Dairy Road interchange;
bridge widening at specific locations and installing new noise barrier walls
at locations between Hollywood Boulevard and Taft Street. Completion Date:
April 2015; $112 Million Palmetto
Expwy Section 5 SR826/SR836 Extension - Design Build, FDOT, District 6,
Miami-Dade County, FL - Safety Manager The work consists of
improvements and reconstruction of the SR826/Flagler Street and SR836/NW 72nd Avenue Interchanges. All existing bridges within the limits of
the project are to be replaced, except the SR836 mainline bridges over the
CSX/FEC railroad bridges, and the widening of the existing SR836 eastbound to
the exit ramp to NW 57th Ave. The project includes new drainage, lighting,
ITS, signalization and signing and pavement marking improvements. This
limited access FDOT D6 project involves the placement of 87,336 CY bridge and
cast-in-place concrete with 17,104,118 lb. reinforcement steel.
Completion Date: March 2015; $559 Million Turnpike Ramp Reconstruction @ PGA
Blvd. , FDOT, Turnpike District, Palm Beach County, FL – Safety
Manager The work consists of providing all labor,
materials, equipment and incidentals necessary for the replacement of ramp
bridge at the PGA Boulevard / Florida's Turnpike Interchange and associated
ramp improvements.
Completion Date: July 2013; $6.1 Million Indiantown Road Bridge Replacement
, FDOT, Turnpike District, Palm Beach County, FL – Safety Manager The work consist of the construction of the new
entrance/exit ramp at Indiantown Road and the Florida’s Turnpike (SR91).
Work includes demolition of the existing bridge and construction of one (1)
new bridge. Structures include FIB 78” prestressed concrete beams over
SR91, MSE walls at each end of the bridge, temporary critical steel pile
walls and temporary critical MSE Wall. Completion Date: March 2013; $4.6
Million SR874 – Killian Parkway
Interchange, Miami Dade Expressway Authority (MDX) Safety Manger - The work consists of materials, equipment and incidentals necessary
for the widening, surfacing, and reconstruction of State Road 874 from North
of Southwest 117th Avenue to
South of Kendall Drive and the roadway and bridge improvements to the Killian
Parkway Interchange. This limited access MDX project involved the
placement of 4,960 CY bridge and cast-in-place concrete with 1,200,000 lb.
reinforcement steel. Completion Date: Jul 2012; $70.6 Million SR112 (I-195) from NW 10th
Ave. to Biscayne Bay, FDOT, District Six, Miami-Dade County, FL – Safety
Manager The work consist of the widening, bridge
repair/rehabilitation on SR 112 (I-195) from NW 10th Avenue to Biscayne
Bay. Work included concrete pavement (new and restoration), retaining
walls (cast in place, soldier pile/lagging, sheet pile and MSE), ITS,
signalization and roadway lighting. Bridges construction included
foundations of steel pipe piles and pre-stressed concrete piles, mass
concrete substructure and Type II, III and IV pre-stressed concrete beams.
The limited space between the right of way and limits of construction served
as the primary challenge on the project. SR-112 (I-195) being a major
thorough fare in urban Miami, proper traffic control and detour planning was
required with not only the Department but with the City. A high
strength early break concrete pavement mix was used for the nighttime pavement
rehabilitation activity which allowed the traffic to travel on it the
following morning. Completion Date: June 2012; $ 19.9 Million Palmetto Expwy Section 2 at Bird
Rd/SR-874/Miller Drive – Design Build, FDOT District 6, Miami Dade County,
FL Safety Manager The work consists of the addition of one general
use lane in each direction; auxiliary lanes between all interchanges;
interchange improvements and, operational and safety improvements along the
SR826 mainline and ramps. The interchanges that are improved are the SR826
interchanges at SW 56 Street/Miller Drive, SR874/Don Shula Expressway and SW
40 Street/Bird Road. Four of the bridges are over the CSX railroad and are
comprised of steel girders. Completion

Street @ Turnpike & Toll Plaza in Miami, FL FDOT District Turnpike –
Safety Manager - Construct express toll lanes with Signature
Tolling Gantries at the existing Okeechobee Mainline Toll Plaza on the HEFT
(SR821). Renovation of existing Toll Plaza. Also new constructions of
the 74th Street interchange with the HEFT.

The work consist of constructing a major bridge
from Kendall Drive (88 St.) over the existing SR878 and tie back into the
Northbound SR874. Due to the confined project area, one of the challenges was
to build in a predominantly residential area between two existing highways. Completion

Expressway – FDOT, District 6, Miami-Dade County, FL – Safety Manager The work consist of traffic operations improvements to the State Road
SR826 (Palmetto Expwy) N.W. 154 Street Interchange bridge widening over
canal. Completion Date: Feb 2007; $3.7 Million SR836 Extension, Design Build, MDX,
Miami-Dade County, FL – Safety Manger The work
consist of the complete design and construction of the SR836 (Dolphin Expwy)
from NW 107th Ave to NW 137
Ave. Construction of nine vehicular bridges., the widening and replacement of
traffic railing barrier at two existing bridges over NW 107th Avenue (bridges 14 & 15), ten MSE retaining
walls and three Sound Walls. Bridges 1, 2, 3, 6, 7, 8, & 9
are comprised of steel longitudinal box girders; steel cross girders, and
flair and circular concrete columns on spread footings. Bridge 13, which
spans NW 107th Avenue,
consists of precast Florida U-Beams, caps, columns and spread footings. The
Bridge at SW 137th Avenue over
Tamiami canal consists of a flat slab bridge, caps, and 18” prestressed
concrete piles foundation. The Sound Walls consist of a system of
precast components assembled over a 30-inch diameter auger cast piles
foundation. Temporary components to the project include support of
embankments with sheet piles and soil anchors. Completion Date: Jan 2007; $43
Million SR836 WB Auxiliary Lanes from 57th
Ave to Palmetto Expressway, MDX, Miami-Dade County, FL – Safety Manager The work consist of an auxiliary lane on the
Westbound 836 (Dolphin Expwy) form NW 57th Ave to a point east of the SR826 (Palmetto Expwy). Build a two
lane bridge adjacent to the existing Westbound bridge, three MSE Walls, a
soil stabilization system which included mini piles and geogrid, 48” storm
drainage, milling and resurfacing, Type V Prestressed Beams and 30”
Prestressed Piles. Due to the confined area, one of the challenges on
this project will be to build the bridge between the two canals and the two
existing railroad tracks. Completion Date: April 2006; $12.3 Million S.W. 87th
Ave. from Bird Road to Tamiami Trail, FDOT, Miami-Dade County, FL –
Safety Manager The
work consist of water main construction, roadway reconstruction and bridge
widening of SR973 (Galloway Road) from SR 976 (Bird Road) to Grand Canal
Drive. Completion Date: Sept 2005; $6.4 Million SR836 Westbound to Southbound HEFT,
MDX, Miami-Dade County, FL – Safety Manger The
work consist of the complete design and construction of the re-aligned
westbound SR 836(Dolphin Expwy) from a point west of SW 97th Avenue to the southbound Homestead Extension of
Florida Turnpike (HEFT). The project consists of four vehicular
bridges, roadway drainage, earthwork, asphaltic concrete roadway, six MSE
retaining walls, retention ponds, roadway lighting, custom overhead sign
structures, and tree relocations. Bridges 11 & 12 are comprised of
steel box beams with integral post-tensioned concrete caps on spread
footings. Bridge 16 which spans NW 107th Avenue consists of Florida U-Beams, caps,
columns and spread footings. Bridge 10 is a widening of the existing
HEFT Ramp over NW Flagler Street consists of AASHTO beams, cap, columns and
footings with 18” pile foundations. Temporary components to the project
include support of embankments with sheet piles and soil anchors, excavation
supports and traffic detours.
